Employer Evaluation Form
Long Term Co-op students must have their employers complete a preliminary (3 month) and a final (12 or 16 month) evaluation.The employer is welcome to use their own employee evaluation form, or the form provided by Engineering Career Services.
The evaluation should be completed by the student's direct supervisor.
